The purpose of this survey is to gain insight from those with lived homelessness experience who have accessed the homeless response and housing systems at the state and county levels in Minnesota. By completing this survey, you are giving consent for your answers to be anonymously shared with the Minnesota Department of Human Services. Your name, contact information, and mailing address will only be used for the purpose of mailing out a gift card in return for completing this survey. By completing this survey, you are giving consent for the American Indian Family Center (AIFC) to send your gift card to the address provided.
